Visa Information (deadline : 8 December, 2017)

Participant who needs the invitation letter should indicate the following information and sent it to: mhd2018@qst.go.jp(visa)

‣ Nationality
‣ Occupation or Name of University, Department, Course (Doctoral or Master)
‣ Name on your passport
‣ Gender
‣ Date of birth and age on the date of visa application
‣ City name where you will apply your visa
‣ Travel Schedule in Japan (Date, Activity plan, Contact (if any), Name of the hotel)
‣ Postal mail address for the documents to be sent
‣ Phone number

＊VISA applicants should be required to send the Registration information as well.
